The name of the lead created using Radikal Form is taken from the rfSubject of the form-the specified subject of the message in the hidden field, or from the hidden TITLE field, depending on which field is the last.

When creating a lead using RadicalForm, the source of the lead and the ID of the responsible employee are taken from the plugin settings.
